Your First Visit

Congratulations on taking your first step toward better health!  Undoubtedly, you have been referred to us by a family member or friend and have some idea of what to expect on your first visit.  The first visit will take approximately 45-60 minutes.  We are looking forward to meeting you and assisting with meeting your health and wellness goals.   

Here are the initial steps needed to become a patient at the clinic:

Paperwork:  Intake Forms  

Our goal is to make your first visit with us comfortable and educational.  Upon arrival, you will be asked to complete the New Patient Intake Forms and to furnish a photo ID and your insurance card (if applicable).  If you have any imaging (x-rays, MRI, etc) please bring these with you to the first visit.  The information you provide will give us important information about your current health condition, previous medical history and more.  Once the forms are completed, you will be escorted to the consultation room.  Dr. Morales will take a detailed medical history to determine if chiropractic care is right for you.

Examination 

After the consultation, Dr. Morales will conduct a thorough physical examination.  This physical examination will include postural, neurological, orthopedic and physical tests to assess the neuro-musculoskeletal system.  The objective findings gained from this examination will allow Dr. Morales to diagnose your condition and select the appropriate recommendations.  During the examination, Dr. Morales will educate you on your condition.  In some cases, a referral for imaging may be given prior to rendering treatment. 

X-Rays/MRI/CT 

If required, Dr. Morales may refer you for imaging, i.e. x-rays, MRI, etc. If this imaging is required, you will be given further instructions.

Treatment Plan/Insurance/Payment 

Once the intake forms, consultation, and examination are complete, Dr. Morales will discuss your treatment options.  With your consent, we will move forward with an individualized treatment plan.  With regards to payment, our office accepts many insurance plans.  We will discuss your individual situation and you will select the payment option that best fits your needs.
